We are improving the way we deliver our services to the community. Developing a new way for you to complete and lodge forms is a key step in that process.
In 2021, we released an online application form (Form F16) and have now completed development of an online Form F17 (employer’s declaration). The Form F17 is the employer’s declaration in support of an application for approval of an enterprise agreement (other than a greenfields agreement). The new online Form F17 allows you to complete your declaration and upload supporting documents online.
The online Form F17 is designed for you to use with the online Form F16. When you apply online, you must complete both forms online.
The online form assists you in lodging compliant declarations by prompting for mandatory information and alerting you if there is incomplete or inaccurate information. The online Form F17 has sharing functionality which allows you to share the form with other organisations or people within your organisation.
We recommend you complete the Form F17 online first and make a note of the reference number. Next, complete the Form F16 online and use the reference number to link the Form F17 to the application. Then, when you lodge the Form F16 both forms will be submitted together.
Our online agreement forms can be accessed on the Commission’s online lodgment service. Just follow the prompts to set up an account and get started. We encourage you to create an account and apply online. Online forms are the preferred lodgment method.
The paper-based Form F16 and Form F17 are no longer available on the Commission’s website. If you cannot apply online, please email us at firstname.lastname@example.org and we’ll be able to assist.
