Use this form in 2 situations: to tell us you have a lawyer or paid agent acting for you or to tell us you will ask permission for them to represent you in a conference or hearing.

Who can use this form
Use this form only if you are a party in a case at the Commission.
Before you start
Have this information ready before you begin. You will need to give us:
- the Commission case ('matter') number, which is in the material we sent you
- your details as the person or organisation giving notice
- details of the lawyer or paid agent (if you are giving us notice that they are acting for the party)
- details of the conference or hearing (if you are giving notice that the party will ask permission for a lawyer or paid agent to take part).

You can also send or deliver your documents to the Commission office near you.
What happens next?
As soon as possible, you must also send (‘serve’) a copy of this notice to all other parties in the case.
