If you are worried you will miss the deadline because you are waiting for information, you can contact us to find out what you can do, or you can apply now with as much information as you have.
Apply online
When you apply you are starting a legal process. You may need to pay an application fee.
You can only make one dismissal application. Use the check eligibility buttons to help choose the right type of application for you.

You can apply online for the following applications:
- Enterprise agreement approval application (other than a greenfields agreement) (Form F16)
- General protections application not involving dismissal (Form F8C)
- Unlawful termination dispute application (Form F9)
- Stop bullying at work order application (Form F72)

From there you can download forms and:
- fill out the form in Word or PDF and email it to us (all forms)
- print the form, then fill it out and post or deliver it to your nearest Commission office (all forms)
