How to lodge forms
The Fair Work Commission’s online case management system eCase is the easiest way to lodge a form.
All the forms to start an application are available in digital form within eCase. You don't need to download these forms before commencing. eCase allows you to save responses in draft so you can review before lodgment.
If you need to use a supplementary form, or are not using eCase, please download the appropriate form from this page. Once you have completed the form you can upload it to eCase.
If you are not using eCase, you can lodge your form at one of our Commission offices:
- by email
- by facsimile transmission
- by post, or
- in hardcopy over the counter.
