Who can use this form
Use this form if you are an employee organisation covered by the agreement and:
- you intend to lodge this declaration with the Form F23H application with the Fair Work Commission, or
- you have received the Form F23HA declaration in relation to the agreement.
Before you start
Have this information ready before you begin. You will need to tell us:
- your details as the person making the declaration
- the name of the agreement that will be varied if approved
- the name of the employer that will be removed from the agreement if variation is approved
- about any statements in the Form F23HA that you disagree with, and why.
You must send (serve) a copy of your declaration to the employer that will be removed from the agreement and each employee organisation that is covered by the agreement.
